Waivers
INDIAN NATIONS YOUTH SPORTS - TERMS & CONDITIONS:
By checking the box below, I the registrant and/or legal guardian of the registrant am legally agreeing to all terms and conditions set forth by Indian Nations Youth Sports and the individual club and/or organization in which I am registering.
I am agreeing that I am at least 18 years of age and the legal guardian of the person/persons registering hereof. I further agree that any information provided in the following pages is truthful and accurate to the best of my knowledge.
I also state that any falsification of documents, illicit or immoral activities will be punished pursuant to the by-laws of the proper governing authorities, and that such activity will lead to penalties including: no reimbursement of funds and partial or permanent expulsion from the affiliation/organization I am applying for.
Right to Refuse Service
By accepting the terms of membership in the Indian Nations Youth Sports (INYS) programs I acknowledge that the INYS is a private organization and that it reserves the right to refuse membership and/or participation to any person whose actions hinder the activities of the INYS and that such determination will be at the sole discretion of Indian Nations Youth Sports.
Arbitration
IT IS AGREED THAT ANY MATTER IN DISPUTE BETWEEN YOU AND INDIAN NATIONS YOUTH SPORTS SHALL BE SUBJECT TO ARBITRATION AS AN ALTERNATIVE TO COURT ACTION PURSUANT TO THE RULES OF THE AMERICAN ARBITRATION ASSOCIATION, A COPY OF WHICH IS AVAILABLE ON REQUEST FROM THE COMPANY. ANY DECISION REACHED BY ARBITRATION SHALL BE BINDING UPON BOTH YOU AND THE COMPANY. THE ARBITRATION AWARD MAY INCLUDE ATTORNEY'S FEES IF ALLOWED BY STATE LAW AND MAY BE ENTERED AS A JUDGEMENT IN ANY COURT OF PROPER JURISDICTION.
Attorney's fees
The Parties agree that if Indian Nations Youth Sports prevails in any litigation or arbitration brought arising from any dispute, then INYS shall be entitled to recover all costs, expenses and attorney’s fees associated therewith.

PARTICIPATION AUTHORIZATION AND RELEASE OF LIABILITY:
For himself/herself and for his/her spouse and for his/her child and other children hereby represents and agrees to the following:
My child has my permission to participate in all sponsored or endorsed activities. The Association and its Board Members and Directors, Officers, Coaches, Coaching Staffs, agents and licensees are hereby released from any and all liability or responsibility for any injury that may occur to my child, to me, to my spouse and any of my other children resulting directly or indirectly from my child's participation in activities including, but not necessarily limited to, league tournaments, practice games, practices, transportation to and from games and tournaments or otherwise and the use of practice facilities, games facilities, concession facilities or any other facility.

AUTHORIZATION FOR EMERGENCY TREATMENT:
In the event of a medical emergency, I hereby authorize my child to be transported to the nearest emergency room or medical facility. I also authorize officials to secure the use of an ambulance for transporting my child to the hospital and/or to administer first aid treatment as necessary. I further authorize any physician, surgeon or dentist of the nearest emergency medical center to administer any emergency treatment procedure or medicine necessary or advisable, when accompanied by an adult. I further agree to pay the hospital, doctors and ambulance fees for all services rendered to the above named child. I request that this authorization remain in force from this date until the end of the calendar year unless notified in writing of a change by me.

RULES AND REGULATIONS CONTRACT:
I have read the Rules and Regulations and Code of Ethics, of which I have a copy, as stated by the Board of Directors . I hereby agree to abide by said rules and regulations. I have also read and fully understand the penalties prescribed for violation or non-compliance of said rules and regulations and by checking the box do agree to these terms and conditions.

REFUND POLICY
If we cannot place your player on a team for any reason, we will refund you the full amount of your registration fee.
Deadlines for refund of attendee registration fees:
• January 1st, 2026 – 80% refund of total registration
• February 1st, 2026 - 50% refund of total registration
• After February 1st, 2026 – No refunds will be given and any unpaid fees will be billed to the registrant.
If, in the sole discretion of the Board of Directors, an extraordinary circumstance occurs, the board may consider a waiver of the cancellation penalty on a case-by-case basis and without precedence. If an attendee feels that he/she has experienced an extraordinary circumstance, he/she should email Bixby Spartan Youth Lacrosse with as much information as possible. The appeal will be brought up at the next available board meeting for discussion and a decision vote. Once a decision is reached, the Board will notify the affected individual.
The board decision will be considered final and there will be no further appeals process.
All refunds will be subject to a 20% processing fee.
Medical Disclaimer: Due to the extreme circumstances caused by the COVID-19 pandemic, which has affected every aspect of the global economy, all refund requests shall be handled on a case-by-case basis. The following refund scenarios will only be applicable to situations beyond the control of Bixby Spartan Youth Lacrosse, INLC, Nienhuis Sports Complex such as, but not limited to: public safety, health concerns and state and/or municipal directives.
a. Refunds due to Event Cancellation - All registrations are non-refundable, unless the event is canceled. Bixby Spartan Youth Lacrosse may, at its sole discretion, withhold a processing fee equal to 20% of the registration price. Bixby Spartan Youth Lacrosse shall issue refunds within sixty (60) days of the event’s scheduled date of completion. Under no circumstances shall Bixby Spartan Youth Lacrosse be obligated to issue a refund to any team that completes half of their scheduled games at a league event. Bixby Spartan Youth Lacrosse reserves the full right to shorten or alter games at its events in order to complete as many games as possible.
In the force majeure event that an outbreak of communicable disease and/or a governmental order prohibiting mass events makes holding an individual event impossible, impracticable, or illegal, Bixby Spartan Youth Lacrosse shall handle all refunds on a case-by-case basis. Bixby Spartan Youth Lacrosse further reserves the right to alter its policy on a case-by-case basis should unusual or extreme circumstances prohibit participation by a properly registered team/player.

PRIVACY POLICY
Protecting private information is our priority. This Statement of Privacy applies to Bixby Spartan Youth Lacrosse and governs data collection and usage. For the purpose of this Privacy Policy, unless otherwise noted, all references to Bixby Spartan Youth Lacrosse, Indian Nations Lacrosse Conference includes BSYL and INLC. Sportabase is a Youth Sports League site. By using Sportabase, you consent to the date practices described in this statement.
Bixby Spartan Youth Lacrosse and INLC may collect personally identifiable information such as your:

- Email Address
- Phone Number
When you register for BSYL and INLC products and services, we collect billing and credit card information. This information is used to complete the purchase transaction.
BSYL and INLC may also collect anonymous demographic information, which is not unique to you, such as your:
- Age
- Gender
We may gather additional personal or non-personal information in the future.
BSYL and INLC encourages you to review the privacy statements of websites that you choose to link from so that you can understand how those websites collect, use and share your information. BSYL and INLC is not responsible for privacy statements or other content websites outside of BSYL and INLC.
BSYL and INLC collects and uses your personal information to operate and deliver services you have requested. BSYL and INLC may also use your personally identifiable information to inform you of other products or services available from BSYL/INLC and its affiliates. BSYL and INLC may also contact you via surveys to conduct research about your opinion of current services or potential new services that may be offered.
BSYL and INLC does not sell, rent or lease it customer lists to third parties.
BSYL/INLC may, from time to time, contact you on behalf of external business partners about a particular offering that may be of interest to you. In those cases, your unique personally identifiable information (email, name, address, telephone number) is not transferred to the third party. BSYL/INLC may share date with trusted partners to help perform statistical analysis, send you email or postal mail, provide customer support or arrange for deliveries. All such third parties are prohibited from using your personal information except to provide these services to BSYL/INLC and they are required to maintain confidentiality of your information.
BSYL/INLC will disclose your personal information, without notice, only if required to do so by law or in the good faith belief that such action is necessary to: a) conform to the edicts of the law or comply with the legal process served on BSYL/INLC or the site; b) protect and defend the rights of property of BSYL/INLC; and c) act under exigent circumstances to protect the personal safety of users of BSYL/INLC or the public.
BSYL/INLC collects personally identifiable information from children under the age of thirteen (13). BSYL/INLC collects this information for the following reasons:
BSYL/INLC collects name, birth date, gender, grade for all athletes who participate in an BSYL/INLC club/town program, including children under the age of thirteen (13). The information collected is used exclusively to verify the grade and age team eligibility and all information collected is provided by a parent/guardian over the age of eighteen (18).
We provide information about our personal date practices for children wherever we knowingly collect personal data from children. If you are under the age of thirteen (13), you must ask your parent or guardian for permission to register for any BSYL/INLC event or league. If you are a parent and you have questions regarding our data collection practices, please contact us using the information provided in your registration email.
We respect your privacy and give you the opportunity to opt-out of receiving announcements of certain information. Users may opt-out of receiving any or all communications from BSYL/INLC by unsubscribing from emails that BSYL/INLC sends for marketing purposes.
BSYL/INLC will occasionally update this Statement of Privacy to reflect company and customer feedback. BSYL/INLC encourages you to periodically review this Statement to be informed of how BSYL/INLC is protecting your information.

PHOTOGRAPHY & VIDEO RELEASE
Allows Bixby Spartan Youth Lacrosse, INLC and INYS to use the appearance (both in video format and still image) and voice of an individual for marketing and promotions; may include use for media, online, physical products (e.g. DVDs), or any other type of publication. Subjects may used for digital and social media, to include our website, Facebook, Instagram, Twitter, etc., as well as, physical and digital flyers.
After the initials of the individual is provided on the document, the rights to their image, video or still, and any likeness of their voice is released to Bixby Spartan Youth Lacrosse INLC and INYS. There will be no compensation provided to the video/photo owner.

CODE OF BEHAVIOR/CONDUCT
Bixby Spartan Youth Lacrosse is committed to providing a safe environment for player participation.
Aggressive, threatening or other inappropriate behavior by members, their families, their friends and other sporting personnel while attending a game or event will not be tolerated.
These behaviors are outlined in our Code of Behavior and specifically include:
• Use of bad language.
• Harassing or ridiculing players, coaches, officials or other spectators.
• Making racist, religious, sexist or other inappropriate comments to players, coaches, officials or other spectators.
• Any threatening behavior or physical altercation between spectators and players, coaches, officials or other spectators.
• Putting undue pressure on children, berating them or putting down their performance.
• Drinking at a game or training or being drunk or under the influence of illegal drugs at an organization event to include practices.
Non-Compliance
Coaches, parents, players and other persons engaged in lacrosse under BSYL found to have behaved inappropriately and who are associate members or have agreed to abide by our club's Code of Behavior and this policy, may face disciplinary action as deemed necessary by the BSYL Board and/or the INLC Commission. Violations of the Code of Conduct will be dealt with swiftly and harshly.
By agreeing, you are acknowledging the aforementioned policy and are subject to disciplinary action up to and including suspension and/or expulsion from the program.

PLAYER ELIGIBILITY & COMMITTMENT POLICY
Players registered with Bixby Spartan Youth Lacrosse are expected to honor their commitment to the program for the duration of the season. Participation with outside lacrosse teams or in lacrosse leagues during the same season creates conflicts of interest, scheduling issues and undermines the cohesion of the team environment BSYL seeks to promote.
To maintain fairness, focus and team unity:
Players are not authorized to participate on two different lacrosse teams during the same INLC season.
Players are not authorized to participate in any other lacrosse league outside of the official INLC spring season while registered as a BSYL player.
This policy ensures that our athletes remain fully committed to their teammates, coaches, and the development model established within the BSYL program.
Players found to be in violation will be dropped from rosters immediately and suspended from further participation with the team and club. NO REFUNDS WILL BE GIVEN!
