INDIAN NATIONS YOUTH SPORTS - TERMS & CONDITIONS:

By checking the box below, I the registrant and/or legal guardian of the registrant am legally agreeing to all terms and conditions set forth by Indian Nations Youth Sports and the individual club and/or organization in which I am registering.

I am agreeing that I am at least 18 years of age and the legal guardian of the person/persons registering hereof. I further agree that any information provided in the following pages is truthful and accurate to the best of my knowledge.

I also state that any falsification of documents, illicit or immoral activities will be punished pursuant to the by-laws of the proper governing authorities, and that such activity will lead to penalties including: no reimbursement of funds and partial or permanent expulsion from the affiliation/organization I am applying for.

Right to Refuse Service

By accepting the terms of membership in the Indian Nations Youth Sports (INYS) programs I acknowledge that the INYS is a private organization and that it reserves the right to refuse membership and/or participation to any person whose actions hinder the activities of the INYS and that such determination will be at the sole discretion of Indian Nations Youth Sports.

Arbitration

IT IS AGREED THAT ANY MATTER IN DISPUTE BETWEEN YOU AND INDIAN NATIONS YOUTH SPORTS SHALL BE SUBJECT TO ARBITRATION AS AN ALTERNATIVE TO COURT ACTION PURSUANT TO THE RULES OF THE AMERICAN ARBITRATION ASSOCIATION, A COPY OF WHICH IS AVAILABLE ON REQUEST FROM THE COMPANY. ANY DECISION REACHED BY ARBITRATION SHALL BE BINDING UPON BOTH YOU AND THE COMPANY. THE ARBITRATION AWARD MAY INCLUDE ATTORNEY'S FEES IF ALLOWED BY STATE LAW AND MAY BE ENTERED AS A JUDGEMENT IN ANY COURT OF PROPER JURISDICTION.

Attorney's fees

The Parties agree that if Indian Nations Youth Sports prevails in any litigation or arbitration brought arising from any dispute, then INYS shall be entitled to recover all costs, expenses and attorney’s fees associated therewith.

PARTICIPATION AUTHORIZATION AND RELEASE OF LIABILITY:

For himself/herself and for his/her spouse and for his/her child and other children hereby represents and agrees to the following:
My child has my permission to participate in all sponsored or endorsed activities. The Association and its Board Members and Directors, Officers, Coaches, Coaching Staffs, agents and licensees are hereby released from any and all liability or responsibility for any injury that may occur to my child, to me, to my spouse and any of my other children resulting directly or indirectly from my child's participation in activities including, but not necessarily limited to, league tournaments, practice games, practices, transportation to and from games and tournaments or otherwise and the use of practice facilities, games facilities, concession facilities or any other facility.

AUTHORIZATION FOR EMERGENCY TREATMENT

In the event of a medical emergency, I hereby authorize my child to be transported to the nearest emergency room or medical facility. I also authorize officials to secure the use of an ambulance for transporting my child to the hospital and/or to administer first aid treatment as necessary. I further authorize any physician, surgeon or dentist of the nearest emergency medical center to administer any emergency treatment procedure or medicine necessary or advisable, when accompanied by an adult. I further agree to pay the hospital, doctors and ambulance fees for all services rendered to the above named child. I request that this authorization remain in force from this date until the end of the calendar year unless notified in writing of a change by me.

RULES AND REGULATIONS CONTRACT:

I have read the Rules and Regulations and Code of Ethics, of which I have a copy, as stated by the Board of Directors. I hereby agree to abide by said rules and regulations. I have also read and fully understand the penalties prescribed for violation or non-compliance of said rules and regulations and by checking the box do agree to these terms and conditions.

SBFA Membership Rules

MEMBER RULES -

Before participating members can participate in practices or games the parent or guardian must do the following:   

Complete and sign all required SBFA forms regarding your child.                    

Participating members must submit and have the following on file with the SBFA: 

Physical form; completed and signed by examining physician.   

Birth Certificate or copy thereof.   

All fees and deposits must be paid in full to SBFA before equipment is issued.

Parents/guardians are required to attend child's games. This is not only to ensure the safety of your child, but also to instill him/her with confidence, support and enthusiasm. Parents are required to sign a parental code of conduct. SBFA is a member organization of Indian Nations Football Conference (INFC). As such, all directors, coaches, staff, parents, spectators and playing participants of SBFA must adhere to all INFC rules and regulations.   

All equipment issued is the sole property of SBFA. All equipment (minus any game uniforms received) must be returned in wearable and/or working order to the SBFA prior to November 17th of the current year. If your child discontinues participation in SBFA, their equipment must be returned within TEN (10) days of such discontinuance. You shall be charged and be required to pay a rental fee of $25.00 after November 17th of the current year. Rental charges will accrue each & every Friday at 12:00pm weekly until such time as the equipment is returned in usable order. Signed rental agreements can be made prior to November 17th of the current year to avoid additional fees. Replacement cost of such uniform and equipment is at the discretion of SBFA. In the event an attorney is employed in the attempt to obtain the return of the uniform and/or equipment rental charges or replacement costs, you shall also be liable for reasonable SBFA attorney fees, cost of litigation and other reasonable expenses.   

Practices shall be at specified days and times designated by the head coach of your team and upon approval of the Board of Directors. Parents must have their child/children to practices and games on or before the designated time and shall pick up their child/children immediately after such practices or games. Practice times are limited to the rules set forth by the INFC. Participants are expected to attend all practices. The last practice before a game is mandatory. Failure to abide by this rule may result in sanctions or loss of play time. The SBFA Board of Directors must approve any variance of this rule.   

Spectators are not allowed on the sidelines or playing surface during games, scrimmages or practices. A coach, official or SBFA Director may waive this rule only for the purpose of carrying out a specified duty.   

SBFA is for our youth and as such, SBFA cannot and will not permit profanity, abusive language, unsportsmanlike conduct, alcohol, drug use or any conduct of a manner that may be deemed offensive. This rule shall apply at all practices, games and events.   

If you incur a grievance you are urged to first discuss the matter with the appropriate coach on your athletes’ team. If not resolved, you should present your grievance to the SBFA Board of Directors in writing.

Football and cheerleading are team sports and require team effort. The coach will have the authority to discipline a player during practice for reasons felt necessary by the coach to maintain order and control of the participants. The coach or coaches may elect to eject a player from the field of practice and/or to sit on the sidelines as a disciplinary measure. If the player is not summoned to return to practice, this shall be called an absence. 50% absences in a one-week period may result in loss of participation at the next game. Absence(s) in practice time may be tallied in days, hours or minutes.   

SBFA encourages the participation of all athletes. As such, persons deemed eligible under specific criteria set forth by the SBFA Board of Directors may obtain full and/or partial scholarships. Applications for funding may be obtained via email at sbfa2022@gmail.com. 

Those seeking scholarships in lieu of registration fees will be asked the following:   

Complete and submit a scholarship application. 

Present documentation as to their financial well-being.   

Upon receiving a scholarship, certain additional duties may be required of the grantee including but not limited to assisting in fundraisers or volunteering at SBFA or INFC events. 

Registration fees for the 2024 season will be set at the following:  

Tackle Football Cost:
Regular season rate of $225 for all returning participants plus an additional $150 uniform for all new players registered, paid and fitted prior to May 7, 2024. A $100 late fee will apply after that date.  

Cheerleading Cost: 
Regular season rate of $180 for all participants plus an additional $280 package is required for all new participants registered, paid and fitted prior to May 9, 2024. 

CHEERLEADERS will cheer with the grade in which they are currently enrolled in school for the 2024 school year. Exceptions to cheer up one grade may be made at the discretion of the SBFA Board of Directors.  

The infraction of a rule or regulation by a participant, parent, guardian or their guest at a game or practice, shall be dealt with by the SBFA Board of Directors and may result in action taken against such offender in one or more of the following:   

Verbal or written reprimand.

Banning said offender from a game or games.

A fine imposed for each offense committed by said offender being a maximum of $250.00 for each offense.   

The removal of an offender or offenders from any and all functions of SBFA for a period of time deemed appropriate by the SBFA Board of Directors.   

Any other such action as the SBFA Board of Directors may deem appropriate.  

SBFA Refund Policy

REFUND POLICY - 

SBFA employs and adheres to the graduated refund policy and rates as set forth below.   

To be considered for a refund the following criteria must be met during the current year in order to obtain any reimbursements of funds.  

Written letter or email containing the following:   

Participants full name   

Grade participating   

Team name (if so placed)   

Parents/Guardians name requesting the refund   

Parents/Guardians phone & email address   

Reason requesting the refund   

Third party date & time stamp on or with the letter at the time of request. A postage stamp is an appropriate stamp if mailed.   

All equipment issued by the SBFA must be returned in working condition.   

All club fundraisers must be completed or returned.   

To meet all final requirements for any refunds, the participant may not engage in any SBFA activities for the remainder of the season once (s)he has quit.   

Refund requests are processed using the third party date/time stamp provided in the submission letter.

Reimbursement amounts once all criteria have been met by parent/guardian requesting a refund.  

SBFA will retain a fee of $25.00 once participants’ online registration is submitted.  

SBFA will retain a fee of $50.00 once participant and/or a parent of the participant check out any SBFA equipment.

SBFA will retain a fee of $125 plus cost of uniform once the participant engages in the drafting process and/or is placed on a team roster. 

SBFA will not issue a refund of any kind once participant engages in any practices.  

SBFA will not issue a refund for uniforms after the order has been placed with vendors.

SBFA Player/Cheerleader Contract

SBFA PLAYER/CHEERLEADER CONTRACT - I do hereby agree to abide by the SBFA player/cheerleader code of conduct as well as the discretionary authority given by authorized coaches in the program. Failure to follow the code of conduct and coaches instruction will be deemed grounds for disciplinary action and/or dismissal from the team/squad. I will respect all coaches at all times. I will work hard in school and stay out of trouble. I will respect and obey my parents/guardians. I will respect referees, opponent players/cheerleaders and coaches at all times. I will have sportsmanship as my number one priority in all games and practices. If I behave in an unsportsmanlike manner it may result in my immediate removal from the game or game suspension. Poor interaction with a referee may result in season long suspension. I will not use profanity or gang talk. I will encourage my teammates in a positive way, regardless of the circumstance. I will attend all scheduled practices and games. If unable to do so, I will contact my coach beforehand. If I have an unexcused absence it may result in loss of playing time. If I have 2 unexcused absences it may result in expulsion from the team. If I have multiple excused absences is it will result in loss of playing time. I will respect, protect and maintain my equipment. I will never throw or purposely drop my helmet. I will return all equipment at years end or immediately upon dismissal from team. I will conduct myself in a way that presents a positive image of the SBFA and myself on and off the field. I will not bully any of my peers. If found bullying it may result in season long suspension.

SBFA Parent/Spectator Contract

SBFA PARENT/SPECTATORS CONTRACT -
This association is bigger than any one player/cheerleader, coach or parent/spectator. As a SBFA parent/spectator we require the following:  

Encourage members of both teams.  

Keep all comments positive.  

Only players and coaches are allowed on the field, no exceptions.  

Leave the coaching to the coaches. If you have a suggestion or would like to coach next year’s team, please email sbfa2022@gmail.com. 

Abusive or foul language and criticizing the officials, coaches, opponents or fans will not be tolerated.

Playing time and positions are at the discretion of the coaches, within the INFC guidelines, and are determined by what is best for the player/cheerleader and the team/squad.  

Football is a team sport and every position is of equal value. Each player will be guaranteed the  number of plays per INFC rules in every game if he/she attends all practices that week.  

You are responsible for all family members and friends attending the games to watch your child.  

Help make this a positive and fun experience for all.  

Report any violations to the SBFA Board of Directors. 

POLICY ENFORCEMENT - 

Individuals may be asked to leave the fields and the player/cheerleader may be removed from the game until the parent/spectator has left.  

Other sanctions as deemed necessary by the SBFA board maybe enforced, Including permanent ban.  

The parent/guardian for the participant grants Indian Nations Youth Sports, Skiatook Bulldogs Football and those associated thereof the right, but does not otherwise impose the obligation, to photograph, videotape and/or otherwise use the parent/guardian and/or his or her participating child’s name, face, likeness, voice and appearance in connection with exhibitions, publicity, advertising and promotional materials, free of charge without reservation or limitation.

Parent Code of Conduct

I hereby pledge to provide positive support, care, and encouragement for my child participating in youth sports by following this Parents’ Code of Ethics: I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and o    cials at every game, practice or other youth sports event. 

I will place the emotional and physical well being of my child ahead of my personal desire to win. 

I will insist that my child play in a safe and healthy environment. 

I will require that my child’s coach be trained in the responsibilities of being a youth sports coach and that the coach upholds the Coaches’ Code of Ethics. I will support coaches and officialls working with my child, in order to encourage a positive and enjoyable experience for all. 

I will demand a sports environment for my child that is free from drugs, tobacco, and alcohol and will refrain from their use at all youth sports events. 

I will remember that the game is for youth - not adults. 

I will do my very best to make youth sports fun for my child. 

I will ask my child to treat other players, coaches, fans, and officials with respect regardless of race, sex, creed, or ability. 

I will help my child enjoy the youth sports experience by doing whatever I can, such as being a respectful fan, assisting with coaching, or providing transportation. 

I will read the National Standards For Youth Sports and do what I can to help all youth sports organizations implement and enforce them. 

Grant of Publicity Rights

The parent/guardian for the participant grants Indian Nations Youth Sports, Skiatook Bulldogs Football and those associated thereof the right, but does not otherwise impose the obligation, to photograph, videotape and/or otherwise use the parent/guardian and/or his or her participating child’s name, face, likeness, voice and appearance in connection with exhibitions, publicity, advertising and promotional materials, free of charge without reservation or limitation.
